CRAB infections in ICUs are on the rise, leading to higher morbidity, mortality, and healthcare costs due to resistance to most antibiotics, including carbapenems. The main resistance mechanisms include carbapenemases, efflux pumps, and changes in the bacterial cell wall.
Current treatments include polymyxins (Colistin, Polymyxin B), which are effective but can lead to resistance, aminoglycosides (Amikacin, Gentamicin), which are limited by resistance, and tetracyclines (Tigecycline, Eravacycline), which are effective against CRAB. Fosfomycin is effective in combination treatments, and combination therapy (e.g., colistin with sulbactam, fosfomycin, or eravacycline) can enhance outcomes.
Previous research shows promise for combination therapies, improving treatment efficacy and reducing mortality. New regimens are being studied to find optimal combinations. Individualized dosing is crucial, considering patient-specific factors like age, weight, and renal function. Adjustments depend on the infection site and comorbidities.
Strict infection control and antimicrobial stewardship programs (ASPs) are essential. ASPs focus on optimizing antibiotic use and reducing resistance through education and surveillance. Cefiderocol and Ampicillin-sulbactam vs. Colistin or Colistin-meropenem for Carbapenem Resistant Acinetobacter Baumannii Bacteremia or Hospital-acquired Pneumonia: Controlled Clinical Study With Historical Controls (CASCADE)

Patients with bloodstream infections, hospital acquired pneumonia or ventilator-associated pneumonia caused by carbapenem-resistant Acinetobacter baumannii (CRAB) treated with cefiderocol combined with ampicillin sulbactam will be compared to patients treated treated with colistin alone or colistin combined with meropenem.

Enhancing antibacterial efficacy and reducing the risk of antibiotic resistance of food-borne pathogen Cronobacter sakazakii by integrating essential oils with silver nanoparticles and antibiotics

Cronobacter sakazakii is known as a food-poisoning bacterium that can withstand stressed environments. This study highlights the urgent need for effective antimicrobial alternatives by examining the emergence of antibiotic-resistant C. sakazakii in Egyptian dietary foods and the effects of essential plant oils and silver nanoparticles. The antibiotic susceptibility profiles of the fifteen C. sakazakii-positive samples were assessed using ten distinct antibiotics. Furthermore, a well diffusion method is used to estimate the minimum inhibitory concentrations of essential oils and silver nanoparticles. Scanning electron microscopy was also used to examine the morphological changes of the powerful antibiotic-resistant strain. Ipenem, ampicillin sulbactam, gentamicin, cefotaxime, ciprofloxacin, amikacin, ceftriaxone, and streptomycin were all resistant to C. sakazakii isolates, with a 100 % resistance incidence. They showed intermediate resistance to trimethoprim/sulfamethoxazole (80 % resistance incidence) and were only sensitive to norfloxacin (26.6 % resistance incidence). Essential oils (12-45 mm) were more successful in suppressing C. sakazakii than susceptible antibiotics (10.83-14.17 mm) and silver nanoparticles (7.17-12.25 mm). Higher inhibitions were provided by mint and basil oils, with mint oil exhibiting the best performance at 45 mm. The combination of antibiotics, silver nanoparticles, and essential oils demonstrated synergistic effects against powerful bacteria. Compared to clove + mint with trimethoprim/sulfamethoxazole (40 mm) or silver nanoparticles with basil oil (35 mm), mint-infused silver nanoparticles showed higher inhibition (45 mm). After being treated with basil oil, C. sakazakii showed morphological changes as seen by scanning electron microscopy. Combining essential oils with antibiotics and silver nanoparticles offers a viable way to counteract C. sakazakii's growing threat. In a medical environment that is becoming more complex, these developments offer hope for improving patient outcomes and safeguarding public health.

Ampicillin-sulbactam versus third-generation cephalosporins in aspiration Pneumonia: A nationwide retrospective cohort study

BACKGROUND:

This study aims to evaluate the clinical outcomes of ampicillin-sulbactam versus third-generation cephalosporins (e.g., ceftriaxone or cefotaxime) in managing aspiration pneumonia.

METHODS:

We utilized the Diagnosis Procedure Combination (DPC) database, a comprehensive national inpatient database in Japan, to identify patients diagnosed with aspiration pneumonia between July 2010 and March 2022. Patients were categorized into two groups based on their treatment: those receiving ampicillin-sulbactam and those receiving third-generation cephalosporins (either ceftriaxone or cefotaxime). To mitigate confounding factors, propensity score overlap weighting analysis was employed to compare in-hospital mortality rates and the incidence of Clostridioides difficile infection between the two treatment groups.

RESULTS:

Among the 548,972 eligible patients, 424,446 received ampicillin-sulbactam, while 124,526 were treated with third-generation cephalosporins. In the third-generation cephalosporin group, 97.7 % of patients were administered ceftriaxone, and 2.3 % received cefotaxime. The mean treatment duration was 8.5 days (standard deviation [SD] 4.3) in the ampicillin-sulbactam group and 7.9 days (SD 4.1) in the third-generation cephalosporin group. Propensity score overlap weighting analysis revealed that patients treated with ampicillin-sulbactam had significantly lower in-hospital mortality (14.6 % vs. 16.4 %; risk difference [RD], -1.8 %; 95 % confidence interval [CI], -2.1 % to -1.5 %; P < 0.001) and a lower incidence of C. difficile infection (2.0 % vs. 2.8 %; RD, -0.8 %; 95 % CI, -0.9 % to -0.7 %; P < 0.001) compared to those treated with third-generation cephalosporins.

CONCLUSION:

Our findings suggest that ampicillin-sulbactam was associated with lower in-hospital mortality and a reduced incidence of C. difficile infection compared to third-generation cephalosporins in patients with aspiration pneumonia.

Microbial Profiles and Antibiotic Resistance in Chronic Otorrhea: An Evaluation of Operated and Non-operated Patients

This study aims to compare the microbial profiles and antibiotic resistance patterns in chronic otorrhea patients with and without surgical intervention, identifying key differences to guide personalized and evidence-based treatment strategies. A retrospective analysis of 120 patients diagnosed with chronic otitis media and otorrhea was conducted. Patients were divided into operated (n = 60) and non-operated (n = 60) groups. Microbial identification and antibiotic susceptibility testing were performed using standard microbiological methods. Statistical analyses assessed differences in microbial prevalence and resistance patterns between groups. Pseudomonas aeruginosa was the most common microorganism in both groups but was significantly more prevalent in the non-operated group (77.8% vs. 53.33%, p < 0.05). Escherichia coli was significantly higher in the operated group (17.78% vs. 3.7%, p < 0.05). Staphylococcus aureus showed no significant difference in prevalence but exhibited resistance exclusively in the operated group. Antibiotic resistance to Ciprofloxacin, Ceftazidime, and Ticarcillin-Clavulanate was high for Pseudomonas aeruginosa in both groups, with no significant differences. Escherichia coli displayed significantly higher resistance to Ampicillin-Sulbactam in the operated group (75.0% vs. 0%, p < 0.05). Operated and non-operated groups showed distinct microbial profiles and resistance patterns. Surgical interventions influence resistance development, particularly in Escherichia coli and Staphylococcus aureus. These findings underscore the importance of routine susceptibility testing to guide effective, individualized treatment and address multidrug-resistant pathogens.
