A Randomized, Single-Blinded, Controlled Study Evaluating Postoperative Non-Opioid Pain Management Utilizing Local Anesthetics Coupled With Modulated Coagulation

This study is a prospective randomized trial examining the effect of tranexamic acid (TXA) and aminocaproic acid (ε-Ahx) with local anesthetics and thrombin in all electively created surgical wound beds in hand surgery to provide long term pain relief and decrease the use of postoperative narcotics.
All surgical created wounds for any hand surgery will be treated in a randomized fashion with the following four arms:
1. Bupivacaine + lidocaine
2. Bupivacaine + lidocaine + thrombin
3. Bupivacaine + lidocaine + thrombin + tranexamic acid
4. Bupivacaine + lidocaine + thrombin + aminocaproic acid
All are applied topically to surgical bed prior to closure of skin.

A Survey Comparing Cardiac Operating Room Setup and Anesthetic Management Among Academic US Adult Cardiothoracic Anesthesiology Programs

OBJECTIVES:

To evaluate contemporary practice patterns in cardiac operating room setup and intraoperative management across academic adult cardiothoracic anesthesiology programs in the United States.

DESIGN:

Cross-sectional survey study.

SETTING:

Accredited US adult cardiothoracic fellowship anesthesiology programs.

PARTICIPANTS:

The survey was emailed to the program directors of the 76 accredited fellowship programs on the Society of Cardiovascular Anesthesiologists website; 51 programs responded (67%).

INTERVENTIONS:

None.

MEASUREMENTS AND MAIN RESULTS:

Responses were analyzed to assess nationwide practices in several common cardiac surgery cases or clinical scenarios. Norepinephrine is the most commonly prepared inotrope and vasopressor for coronary artery bypass graft. Nitroglycerin and nicardipine are the most commonly prepared vasodilators during setup for coronary artery bypass graft. In left and right ventricular dysfunction, epinephrine is the most commonly used first-line inotropic agent. Bivalirudin is the preferred heparin alternative in the setting of heparin-induced thrombocytopenia. Forty percent of programs report routinely performing regional anesthesia during cardiac surgery. Antifibrinolytic use is nearly evenly split nationwide between tranexamic acid and ε-aminocaproic acid. Most institutions use inhaled nitric oxide intraoperatively as a pulmonary vasodilator. Propofol is the preferred sedative, followed by dexmedetomidine for intensive care unit transport of patients.

CONCLUSIONS:

Contemporary practice patterns among academic cardiothoracic anesthesiology programs in the United States can be inferred from the data gathered in this nationwide survey. Equipped with this, institutional and individual self- assessment of current practices can lead to targeted quality improvement and further investigation for optimal strategies in cardiac anesthesia.

Surface-associated phosphoglycerate kinase (PGK) of Staphylococcus aureus binds human plasminogen in a partially lysine-independent manner

Staphylococcus aureus, a potent opportunistic pathogen causes a variety of infections ranging from mild skin infections to life-threatening diseases. With the rapid increase in antimicrobial resistant pathogens, it is critical to comprehensively understand the virulence strategies employed by the pathogen to invade the host. The glycolytic enzymes of S. aureus have been reported to possess additional roles promoting pathogenesis. In this study, we cloned, overexpressed and purified the S. aureus glycolytic protein, phosphoglycerate kinase (PGK) (EC 2.7.2.3) and investigated its localization and interaction with human plasminogen. To elucidate the localization of PGK, a whole cell ELISA and cell fractionation was performed. We report the localization of the wild type protein in the cytosol as well as the cell surface associated fractions of S. aureus. Whole-cell ELISA with anti-PGK antibodies showed significant antibody binding compared to control with secondary antibody (∼11-fold increase, p = 0.0005), indicating surface exposure of PGK. Purified recombinant PGK bound human plasminogen (p < 0.0001) and enhanced its conversion to its active form in the presence of tissue plasminogen activator (p < 0.0001). Plasminogen binding to PGK was only partially inhibited by ε-aminocaproic acid, indicating that lysine-mediated interactions are not the sole determinants. These results were fully corroborated with the molecular dynamics which revealed stable binding throughout the entire 500ns period and a similar disposition of the lysines. These findings suggest that PGK may function as a moonlighting protein, playing an essential role in the interaction of S. aureus with its host through plasminogen binding.

Successful Perioperative Management Strategies in Surgical Correction of Craniosynostosis for Patients With von Willebrand Disease

This study evaluates the rate of von Willebrand disease (vWD) in patients with craniosynostosis and describes the management of patients with vWD who require surgical correction of craniosynostosis (SCC). This is a retrospective cohort study of 190 consecutive patients who underwent initial SCC at a university-affiliated community hospital between January 2016 and May 2024. Before surgery, patients were evaluated by the Pediatric Blood Management Service and underwent laboratory tests for anemia and vWD. Patients who screened positive for vWD received a hematology and oncology (Heme/Onc) referral, preoperative infusion of antihemophilic factor/von Willebrand factor complex (Humate-P), and postoperative administration of aminocaproic acid (Amicar). Univariate analysis was used to compare transfusion volumes between patients with and without vWD. A total of 13.2% of patients were referred to Heme/Onc due to abnormal vWD labs, and 6.8% of patients were ultimately diagnosed with vWD by Heme/Onc. All patients diagnosed with vWD received Humate-P preoperatively, and 77% of patients with vWD also received postoperative Amicar. Compared with all other patients, patients with vWD demonstrated no difference in estimated blood loss (EBL) or intraoperative and total pRBC volumes. There was also no difference in EBL nor pRBC volumes when comparing patients with vWD to anemia protocol-adherent and relative anemia protocol-adherent cohorts. vWD in our craniosynostosis population was higher than in the general American population, which is ∼1%. Preoperative screening for vWD and appropriate perioperative management can effectively address blood loss and transfusion needs for patients undergoing SCC.
