SummaryLisinopril is a type of medication known as an angiotensin-converting enzyme (ACE) inhibitor that is frequently prescribed for the management of hypertension, heart failure, and other cardiovascular conditions. Optimal Management of HIV Infected Adults at Risk for Kidney Complications in Nigeria

In this study, the Investigators plan to determine the optimal means to prevent or slow the progression of kidney disease among genetically at-risk northern Nigerian HIV-infected adults. Based on data from studies of diabetic kidney disease that used medications that block the renin angiotensin aldosterone system (RAAS), we plan to evaluate whether or not RAAS inhibition (using a widely available medication that blocks RAAS) in HIV-infected adults produces similarly promising results.

Prenatal Care Initiation and Exposure to Teratogenic Medications

Importance:

With new legal abortion restrictions, timing of prenatal care initiation is critical to allow for discussion of reproductive options among pregnancies exposed to teratogenic medications.

Objective:

To investigate the prevalence of prenatal exposure to teratogenic medications and prenatal care initiation across gestational weeks.

Design, Setting, and Participants:

This descriptive, population-based cross-sectional study used health encounter data from a national sample of individuals with employer-sponsored health insurance. A validated algorithm identified pregnancies among persons identifying as female that ended with a live or nonlive outcome between January 2017 and December 2019. Data were analyzed from December 2022 to December 2023.

Exposures:

Prenatal exposure to any of 137 teratogenic medications, measured via pharmacy and medical claims. Measurement of prenatal care initiation was adapted from the Children’s Health Care Quality Measures.

Main Outcomes and Measures:

Prevalence of prenatal exposure to teratogens and prenatal care initiation by gestational week. Timing of prenatal teratogenic exposure was compared with timing of prenatal care initiation and legal abortion cutoffs.

Results:

Among 639 994 pregnancies, 472 472 (73.8%; 95% CI, 73.7%-73.9%) had a live delivery (mean [SD] age, 30.9 [5.4] years) and 167 522 (26.2%; 95% CI, 26.1%-26.3%) had a nonlive outcome (mean [SD] age, 31.6 [6.4] years). Of pregnancies with live deliveries, 5.8% (95% CI, 5.7%-5.8%) were exposed to teratogenic medications compared with 3.1% (95% CI, 3.0%-3.2%) with nonlive outcomes. Median time to prenatal care was 56 days (IQR, 44-70 days). By 6 weeks’ gestation, 8186 pregnancies had been exposed to teratogenic medications (25.2% [95% CI, 24.7%-25.7%] of pregnancies exposed at any time during gestation; 1.3% [95% CI, 1.3%-1.3%] of all pregnancies); in 6877 (84.0%; 95% CI, 83.2%-84.8%), prenatal care was initiated after 6 weeks or not at all. By 15 weeks, teratogenic exposures had occurred for 48.9% (95% CI, 48.4%-49.5%) of all teratogen-exposed pregnancies (2.5% [2.4-2.5] of all pregnancies); prenatal care initiation occurred after 15 weeks for 1810 (16.8%; 95% CI, 16.1%-17.5%) with live deliveries and 2975 (58.3%; 95% CI, 56.9%-59.6%) with nonlive outcomes. Teratogenic medications most used within the first 15 gestational weeks among live deliveries included antiinfectives (eg, fluconazole), anticonvulsants (eg, valproate), antihypertensives (eg, lisinopril), and immunomodulators (eg, mycophenolate). For nonlive deliveries, most antihypertensives were replaced by vitamin A derivatives.

Conclusions and Relevance:

In this cross-sectional study, most exposures to teratogenic medications occurred in early pregnancy and before prenatal care initiation, precluding prenatal risk-benefit assessments. Prenatal care commonly occurred after strict legal abortion cutoffs, prohibiting consideration of pregnancy termination if concerns about teratogenic effects arose.

Captopril to Lisinopril Conversion in Pediatric Cardiothoracic Surgery Patients Less Than 7 Years of Age (RISE-7)

Hypertension after cardiothoracic surgery is common, often requiring pharmacologic management. The recommended first-line antihypertensives in pediatrics are angiotensin converting enzyme inhibitors. Captopril and enalapril are approved for infants and children; however, lisinopril is only approved for > 7 years of age. This study evaluated safety and efficacy of converting from captopril to lisinopril in patients utilizing a pre-defined conversion of 3 mg captopril to 1 mg lisinopril. This was a single center, retrospective study including patients less than 7 years of age admitted for cardiothoracic surgery who received both captopril and lisinopril from 01/01/2017 to 06/01/2022.The primary outcome was mean change in systolic blood pressure (SBP) from baseline for 72 h after conversion of captopril to lisinopril. A total of 99 patients were enrolled. There was a significant decrease in mean SBP (99.12 mmHg vs 94.86 mmHg; p = 0.007) with no difference in DBP (59.23 mmHg vs 61.95 mmHg; p = 0.07) after conversion to lisinopril. Of the 99 patients who were transitioned to lisinopril, 79 (80%) had controlled SBP, 20 (20%) remained hypertensive, 13 (13%) received an increase in their lisinopril dose, and 2 (2%) required an additional antihypertensive agent. There was a low overall rate of AKI (3%) and hyperkalemia (4%) respectively. This study demonstrates that utilizing lisinopril with a conversion rate of 3 mg of captopril to 1 mg of lisinopril was safe and effective for controlling hypertension in pediatric patients following cardiothoracic surgery.

Population pharmacokinetics of lisinopril in hypertensive children and adolescents with normal to mildly reduced kidney function

Aims:

Lisinopril, an angiotensin‐converting enzyme inhibitor, is a frequently prescribed antihypertensive drug in the paediatric population, while being used off‐label under the age of 6 years in the USA and for all paediatric patients globally. The SAFEPEDRUG project (IWT‐130033) investigated lisinopril pharmacokinetics in hypertensive paediatric patients corresponding with the day‐to‐day clinical population.

Methods:

The dose‐escalation pilot study included 13 children with primary and secondary hypertension who received oral lisinopril once daily in the morning; doses ranged from 0.05 to 0.2 mg kg−1. Patients were aged between 1.9 and 17.9 years (median 13.5 years) and weight ranged between 9.62 and 97.2 kg (median 53.2 kg). All data were analysed using Monolix version 2020R1 (Lixoft, France) and R version 3.6.2.

Results:

A 1‐compartment model with first‐order absorption and first‐order elimination optimally describes the data. Parameter estimates of absorption rate constant (0.075 h−1 [0.062, 0.088], typical value [95% confidence interval]), volume of distribution (31.38 L 70 kg−1 [10.5, 52.3]) and elimination clearance (24.2 L h−1 70 kg−1 [19.5, 28.9]) show good predictive ability. Significant covariate effects include total body weight on elimination clearance, and distribution volume and estimated glomerular filtration rate (eGFR) on elimination clearance. The effects of eGFR on the elimination clearance are optimally described by a linear effect centred around 105 mL min−1 1.73 m−2. The effects of body weight were implemented using fixed allometric exponents centred around an adult weight of 70 kg.

Conclusion:

Lisinopril dose and regimen adjustments for paediatric patients should include eGFR on top of weight adjustments. An expanded model characterizing the pharmacodynamic effect is required to identify the optimal dose and dosing regimen.

Several Drugs Linked to Risk for Aneurysmal Subarachnoid Hemorrhage

THURSDAY, June 6, 2024 -- Several commonly prescribed drugs are associated with aneurysmal subarachnoid hemorrhage (aSAH), according to a study published online June 5 in Neurology . Jos P. Kanning, from the University Medical Center Utrecht in the Netherlands, and colleagues conducted a drug-wide association study to examine the association between commonly prescribed drugs and aSAH incidence. Use of commonly prescribed drugs was examined across exposure windows (current, within three months; recent, three to 12 months; and past, >12 months). Exposure to 205 commonly prescribed drugs was investigated among 4,879 aSAH cases and 43,911 matched controls. The researchers found similar trends for lisinopril and amlodipine, with a decreased risk for aSAH in association with current use, while the risk for aSAH was increased with recent use. The risk for aSAH was reduced with current use of simvastatin, metformin, and tamsulosin. Increased aSAH risk was seen in association with current use of warfarin, venlafaxine, prochlorperazine, and co-codamol. "Future research should use a more hypothesis-driven approach to further investigate these associations and differentiate between drug class and specific drug substance effects," the authors write. "In addition, this research may help identify additional risk factors for aSAH, potentially leading to new pharmacologic therapy options for aneurysmal management." Could taking certain drugs reduce risk of ruptured brain aneurysm?

A new study suggests that people who take a few common drugs may have a decreased risk of having a bleeding stroke due to a ruptured brain aneurysm. The results do not prove that these drugs reduce the risk of this type of aneurysm; they only show an association. A new study suggests that people who take a few common drugs may have a decreased risk of having a bleeding stroke due to a ruptured brain aneurysm. The study is published in the June 5, 2024, online issue of Neurology®, the medical journal of the American Academy of Neurology. The results do not prove that these drugs reduce the risk of this type of aneurysm; they only show an association. "We urgently need new ways to prevent this type of stroke, which occurs at younger ages and with a higher death rate than other types of stroke," said study author Jos Peter Kanning, MSc, of University Medical Center Utrecht in the Netherlands. "Our current surgical treatments for brain aneurysms have a risk of permanent disability and death that often outweighs the potential benefits, so preventing rupture with a non-invasive drug would be very beneficial." For the study, researchers looked at 4,879 people who had ruptured brain aneurysms, called aneurysmal subarachnoid hemorrhages. Those people were each compared to nine people of the same age and sex, for a total of 43,911 people who did not have ruptured brain aneurysms. Then researchers looked at electronic health records to see what prescription drugs people took. They found that four drugs were associated with a decreased risk of having a ruptured brain aneurysm: the high blood pressure drug lisinopril; the cholesterol drug simvastatin; the diabetes drug metformin; and the drug tamsulosin, prescribed for enlarged prostate. After adjusting for other factors that could affect the risk of stroke, such as high blood pressure, smoking, alcohol abuse and total number of other health conditions, researchers found that people currently taking lisinopril were 37% less likely to have a ruptured brain aneurysm than those not taking the drug. People taking simvastatin were 22% less likely to have a stroke. Those taking metformin were 42% less likely to have a stroke and those taking tamsulosin were 45% less likely. Researchers also found an increased risk of having a ruptured brain aneurysm for people taking four drugs: the blood thinner warfarin; the antidepressant venlafaxine; the antipsychotic and antiemetic drug prochlorperazine; and the painkiller co-codamol. "Future research is needed to investigate these associations and determine whether these drugs are effective in reducing the risk of hemorrhagic stroke," Kanning said. "This work could also help us identify additional risk factors for subarachnoid hemorrhage, potentially leading to new therapies to manage aneurysms." A limitation of the study was that researchers looked at drug prescriptions. It is possible that people may not take their drugs or may take them incorrectly. The study was supported by the European Research Council.
