The purpose of this research is to study whether adding an FDA approved medication called aprepitant (a medication given to patients like you to reduce or eliminate nausea and vomiting) to the usual treatment to prevent nausea and vomiting given to patients during knee replacement surgery helps more patients go home on the same day of surgery (before midnight) by eliminating nausea and vomiting.

The prognosis for relapsed/refractory acute lymphoblastic leukemia (R/R ALL) remains dismal due to intrinsic and acquired chemoresistance, highlighting an urgent need for novel therapeutic strategies. The neurokinin-1 receptor (NK-1R) represents an emerging oncological target, yet its role and therapeutic potential in ALL are incompletely understood. Here, we identify the NK-1R as a robust therapeutic target that is significantly overexpressed in both primary and R/R ALL. The FDA-approved NK-1R antagonist Aprepitant potently inhibited the proliferation of ALL cells and overcame resistance to chemotherapeutic drugs in both patient-derived samples and doxorubicin-resistant cell lines. Mechanistically, Aprepitant triggered a novel mitochondria-driven necroptotic pathway, distinct from the apoptosis induced by NK-1R blockade in other cancers we previously reported [5,6]. This pathway is initiated by endoplasmic reticulum (ER) calcium release and ER stress, leading to mitochondrial calcium overload, reactive oxygen species (ROS) burst, and subsequent mitochondrial fission, which ultimately activates the phosphorylation of the core necroptotic effectors RIP1, RIP3, and MLKL. Pharmacological inhibition or genetic knockdown of key regulators of necroptosis, ER stress, or mitochondrial ROS significantly attenuated Aprepitant-induced cell death. Importantly, Aprepitant exhibited significant in vivo anti-leukemic efficacy in a xenograft mouse model. Our findings not only elucidate a unique, mitochondria-mediated necroptotic mechanism engaged by NK-1R inhibition but also provide a compelling rationale for the immediate clinical repurposing of Aprepitant as a targeted therapy for R/R ALL.

Ameliorative impact of aprepitant on cisplatin testicular toxicity: Dual regulation of PI3K/AKT pro-survival signaling and Bax/cytochrome c/caspase-3 apoptotic signaling

AIM:

Cisplatin is a powerful antineoplastic agent utilized for the management of several cancers. Due to its significant deleterious impact on the testis, manifested as long-term infertility and irreversible azoospermia in 50% of individuals, its clinical application is markedly restricted. Therefore, this study focused on the investigation of aprepitant's potential effectiveness against cisplatin-induced testicular injury due to its previously reported efficacy in modulation of inflammatory, pro-survival, and apoptotic signaling.

MAIN METHODS:

In a 10-day experimental protocol, male Wistar rats were assigned to five groups: normal control, aprepitant (20 mg/kg), cisplatin (10 mg/kg, 5^th day-only), aprepitant (10 mg/kg) + cisplatin, and aprepitant (20 mg/kg) + cisplatin, then sacrificed on day 11. Sperm quality and testosterone level were evaluated, along with biochemical, immunohistochemical, and histopathological analysis of the testicular tissue.

KEY FINDINGS:

Cisplatin administration notably diminished sperm viability, count, morphology, and increased abnormalities, alongside the decline in serum testosterone level. Furthermore, cisplatin upregulated nuclear factor-kappa B p65 (NF-κB p65), while downregulating phosphorylated-phosphatidylinositol-3-kinase (p-PI3K), and phosphorylated protein kinase B (p-AKT) protein expressions in testicular tissue sections. That in turn enhanced the transcription of interleukin-6 (IL-6), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) pro-inflammatory cytokines, aggravated oxidant/antioxidant imbalance, triggered apoptosis via Bax/cytochrome c/caspase-3 intrinsic pathway, while suppressing the antiapoptotic Bcl-2 protein. In contrast, aprepitant treatment guarded against these alterations in a dose-dependent manner.

SIGNIFICANCE:

Aprepitant exhibits efficient protection against cisplatin-induced testicular toxicity that could be attributed to its inhibitory influence on Bax/cytochrome c/cleaved caspase-3 apoptotic signaling, with simultaneous enhancement of PI3K/AKT pro-survival signaling.

Encouraging Progress In Our Dupuytren’s Research

Ventoux Bio July 22, 2026 Dupuytren's Information, Events We have now studied VEN-201 in human Dupuytren’s tissue from several individual donors, and the early results are genuinely encouraging. Across these samples, VEN-201 has shown consistent signs of acting on the biological processes that drive the disease, exactly the direction we hoped to see. What gives us particular confidence is that consistency. Seeing the same direction of effect across tissue from different people, despite the natural variation from one person to the next, is an important early signal that we may be engaging the true drivers of the disease rather than seeing a one-off result. It is still early, and there is real work ahead before this reaches patients, but this is the kind of human-anchored evidence that strengthens our conviction and helps open doors with research and funding partners. Out of respect for the scientific process, we are sharing these findings at a high level for now, with more detail to follow as the work matures. Our focus now is to build on these findings: studying additional donor tissue to further test what we are seeing, deepening our understanding of how VEN-201 works, and advancing the preparation needed to move toward a first clinical study. Each of these steps brings us closer to testing VEN-201 in patients, and each one depends on continued support. This is the question closest to my own heart and the one I receive the most from fellow patients. Our current estimate is roughly 18 months to a first clinical study, and reaching that milestone depends heavily on funding. Here is the honest part. Dr. Satish received her Ph.D. from a prestigious institute in India and did her post-doctoral training at the University of Pittsburgh. Currently, Dr. Satish serves as a faculty at the Division of Asthma Research, Cincinnati Children’s Hospital; named as the top hospital in the US. Daiva Bajorunas MD is an endocrinologist with more than 25 years of experience in the biopharmaceutical industry. She has a strong interest in advancing therapeutic options for conditions of significant unmet medical need. The authorization supports the continued advancement of Hoth's randomized, placebo-controlled, dose-ranging Phase 2a study evaluating HT-001 for the treatment of dermatologic toxicities associated with epidermal growth factor receptor (EGFR) inhibitor therapies. Preliminary data from the ongoing study indicate: Patients achieved the primary efficacy endpoint, with rash severity reduced to clinically manageable levels (ARIGA ≤1) by Week 6 Over 65% of patients reported meaningful reductions in pain and itching Zero patients required dose reduction or discontinuation of their underlying EGFR inhibitor cancer therapy Favorable safety profile, with no treatment-limiting adverse events observed These findings suggest that HT-001 may enable patients to remain on life-saving cancer therapies longer, addressing a major limitation of current oncology treatment regimens.
